Case Management


Access Care is proud to offer conflict-free case management services to those on the HCB waiver. In addition to English speaking, we have a large number of multicultural cases we serve in this department. We currently have bilingual case managers who speak Spanish and Bosnian and have the resources to accommodate other languages as well.

Keeping great communication with other providers is very important to us. This type of communication allows us to better serve our participants, making sure they receive the best possible care and services. Our case managers check in with participants every month via phone and meet with them in person throughout the year. They are very knowledgeable regarding services and will always ensure the participant is receiving the best care.
