How much does it cost to attend the day center?

A.

Our program is funded by Medicaid and most participants receive our services for FREE through the Home and Community Based Waiver (HCB). We have staff that can assist to see if you may qualify for Medicaid and can assist you in applying. If you already have Medicaid, they can see if you qualify to be on the Home and Community Based Waiver and can assist you.

We have a private pay option for clients that do not qualify to be on the HCB waiver. Our day center cost $68/ day, this includes all activities, appointment scheduling, field trips, and meals.

What will my loved one be doing there?

A.

While attending the day center, your loved one will receive personalized care based on their plan of care. We have a physical therapist that does exercises with our clients every morning, our kitchen prepares breakfast, our activity coordinators prepare 3 activities at each given time, which include art classes, English classes, crafts, social activities, field trips, etc. Our drivers also transport the clients to and from their scheduled doctor’s appointments throughout the day. Our kitchen prepares lunch, and the clients participate in a few more activities before being transported home.

Adult Day and Healthcare, Attendant Care services, Case management, Home Delivered meals, Transportation, and Primary Care.

We have a private pay option for clients that do not qualify to be on the HCB waiver. Attendant Care services cost $24/ hour.

Who will be taking care of my loved ones?

A.

Each client is assigned an activity coordinator that cares for them throughout the day. We also have medical staff, such as nurses, medical assistants, and certified nursing assistants, that assist with caring for the patients. Every staff member at Access Care is trained in CPR and First Aid, has a clean background check, has training on abuse and neglect, and training from the department of aging and independent living to best serve of our clients.

Do you have transportation services?

A.

Yes, Access Care has a fleet of buses and smaller vehicles to transport our clients to and from their homes and to medical appointments. If clients are approved for transportation services through the Federated Transportation Services of the Bluegrass, then our transportation services are free! If not, we do have a private pay option with a set amount of $25 for each trip. Please contact our transportation department if you have further questions 502-891-0029 Ext. 118.
